Chlamydia case investigation across 8 local health departments. Evaluated June 2024 through December 2025 by a CDC EIS Officer under CDC surveillance evaluation guidelines.
Chlamydia is one of the most reported infectious diseases nationally (~1.6 million cases/year). Traditional phone-based investigations faced low contact rates, poor documentation of sensitive behavioral data, and constrained investigator capacity.
The Approach
Replaced or supplemented phone outreach with automated text-based self-report, working in the background while investigators focused on higher-priority cases. Investigators reported Connect "working in the background" — freeing staff capacity for urgent cases without sacrificing data quality.
