Real results from real deployments.

Connect is production-ready and actively deployed. These results come from live implementations across local health departments, military treatment facilities, and community-based programs.

What People Are Saying

“Connect is easy to use, covers multiple functions in one program — case investigation and monitoring — and saves us time. People.Health has been great to work with.”
— Laura B., Public Health Nurse & PCH Supervisor, Barry-Eaton District Health Department

Public Health Surveillance

Chlamydia case investigation across 8 local health departments. Evaluated June 2024 through December 2025 by a CDC EIS Officer under CDC surveillance evaluation guidelines.

Chlamydia is one of the most reported infectious diseases nationally (~1.6 million cases/year). Traditional phone-based investigations faced low contact rates, poor documentation of sensitive behavioral data, and constrained investigator capacity.

The Approach

Replaced or supplemented phone outreach with automated text-based self-report, working in the background while investigators focused on higher-priority cases. Investigators reported Connect "working in the background" — freeing staff capacity for urgent cases without sacrificing data quality.

  • Investigation completion rate: 38% vs. 19.9%
  • Documentation rates: 91.3% vs. 62.7%
  • Time to overall investigation completion improvement: 13 hours after implementation
  • Sensitive behavioral data capture: Significant increase (sexual risk history, anonymous partners)

Investigation Analytics

  • Completion: 38% vs 19.9%
  • Documentation: 91% vs 62.7%

Completion Rate by LHD

  • Genesee County: 45%
  • Ingham County: 38%
  • Kalamazoo County: 35%
  • Washtenaw County: 32%

What People Are Saying

“When I started I had over 1,600 overdue pap smears. Calling each one took nearly six months. Now I send them all at once and my phone starts ringing immediately. Six months of work done in less than three hours — they call me, I don't call them.”
— Anonymous, Population Health Nurse, Military Health System
